Diagnostic Rule: Malignant acini lose their basal layer (Basal Markers negative) and typically overexpress AMACR (+). Benign glands maintain an intact basal layer (Basal Markers positive) and are AMACR negative. HGPIN exhibits both AMACR positivity and an intact/patchy basal layer.
